At Immaculate Heart Home Care, we are determined to provide the best service customized to your needs or the needs of your loved one. Our services span the care continuum, from providing companionship and personal care to providing support for Alzheimer and hospice care – our care team is here to help every step of the way.
To determine what services would be ideal for you, we provide a free in-home assessment that helps us gather information about your daily routines. We will learn about you or your loved one’s lifestyle, medical conditions, personality and the kind of assistance needed that will help them to remain in their home and independent.
Our complimentary assessment will enable us to write a Service Plan. The Service Plan is a customized comprehensive plan to meet your loved one’s psychological, social, safety, and physical needs. We fully explain the services available, answer any questions, and help with the important care decisions faced by the family and loved one in need. Then the best caregiver is matched to the senior’s needs and follow up supervision ensures that the plan is well-implemented. We’ll talk about the best payment options for your family.
We will monitor the Service Plan and suggest adjustments as needs change to ensure that your loved one remains safe, happy, and well cared for.
As the population begins to age, there are more and more people every day that can benefit from our services. Most people do not understand that there is an alternative to moving out of their home and into an assisted living facility, nursing home, or other institution. The goal of end of life care is not rehabilitation but to support individuals as they wish until they pass away. Immaculate Hearts Caregivers are equipped to provide assistance for activities of daily living that are not covered by insurance companies like Medicare or MediCal. Our caregivers are able to help with Homemaking (cooking, light house cleaning, laundry, changing bed linens), Companionship (shopping & errands, safety & security, prompting & reminding), Personal Care (bathing, changing briefs, dressing, repositioning if bed-bound) and Medication Reminders
Been a caregiver can be exhaustive and difficult. About 1 in 3 adults provide informal care to another adult like an aging parent or a disabled relative. It is important for the caregiver to have a plan in place to cope with caregiver fatigue and burnout. This is where we come in.
